Life Care of Wilbraham

Description of resource: Life Care Center of Wilbraham is a skilled nursing facility that utilizes a multi-disciplinary approach to care, focused on providing a full-range of services to enhance quality of life for residents who require long-term care. Life Care Center of Wilbraham provides the following end-of-life care services: Hospice, quality nursing care, social services, activity involvement, and spiritual care, all in a supportive and nurturing environment. Our Spiritual Care Program is an integral part of our end-of-life services. Our Spiritual Care Director works closely with members of the health care team for an integrated approach and is able to provide the following support services: Presence and listening skills; contact with clergy and community members; the Eucharist or sacraments though visiting clergy; scripture or prayers; music or enrichment thorough art; and grief counseling.
