Description of resource: 24 Hours Care offer Staffing (RN, LPN, CNA, HHA) for medical as well as Home Health care for elderly or people suffering from chronic diseases by keeping patients out of the hospital/nursing home environment, and allow them to live in their own homes despite their clinical conditions and medical needs. We specialize in helping patients with ALS, Alzheimer’s, Multiple Sclerosis, Diabetes, Parkinson’s, Mental health, Cancer, Heart Disease + Stroke and Aphasia. We have experienced and trained healthcare professionals who provide compassionate care and comfort to patients suffering from these progressive diseases & make their life a little easier. We take the time to evaluate the needs of each patient and their family and develop a Care plan that offers the best care possible. The Mission of our Agency, is to provide comprehensive, high quality home care services to our patients by creating strong partnerships with them, their families, case managers, discharge planners and physicians. We believe in creating a team of quality, well trained professionals whose goal is the exceptional care and support of our patients.
