Steward Home Care-Hospice

Description of resource: Steward® Hospice can provide the care needed to maximize quality of life, minimize pain and provide support during this transitional time for you or a loved one. Hospice provides a compassionate care team for the patient and his or her family and friends. Our goal is to support you, whether at home or in an assisted living facility, nursing home or hospital. And we bring the same coordinated, collaborative care that is a hallmark of Steward Health Care. Our hospice care services offers each patient and family with a multidisciplinary team of professionals that includes nurses, social workers, home care aides, pastoral care providers, and volunteers who, together with the patient’s physicians, design a program specific to the needs of the patient and family. The goal of hospice care is to provide physical, emotional and spiritual comfort, and increase the patient’s quality of life to its highest possible level. We also give support to the caregivers as they are an intergral component during end-of-life care.
