Our experienced staff of Life Care Managers offer highly personalized services, acting as an essential link between our clients and the services they need. At LifeCare Advocates, we:

• Evaluate - mental and physical functioning through in-home assessments
• Coordinate - home care and home health services
• Guide - in selecting care and housing options
• Review - legal and financial needs; refer to appropriate specialists
• Monitor - care of clients under guardianship
