Description of resource: AseraCare Hospice provides highly skilled and compassionate medical, emotional, and spiritual support to patients and families coping with life-limiting illness. We serve patients and families in skilled nursing facilities, assisted living centers, hospitals, and in their homes in the community. Our interdisciplinary team of nurses, social workers, chaplains, home health aides, and volunteers work closely with you and your physician to create an individualized care plan to maximize your comfort, dignity, and choice.
